Inspector Notes

This monitoring inspection was initiated by licensing staff and concluded on 10/26/2023, between the hours of 9:30 am to 12 pm. There were 38 children present, ranging from infant to preschool-age, with 7 staff supervising, across 4 classrooms. The children were engaged in the following: center activities such as games, Legos, housekeeping play, reading nook; outdoor play, bathroom breaks, hand washing, lunch, infant feeding, diapering, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 5 children records, and 12 staff records were reviewed.

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the center.

Standard 8VAC20-780-130-E
Based on review, 1 of 5 children files indicated that the center did not obtain documentation of additional immunizations once every six months for a child under the age of two years.

Evidence -
1. On the date of inspection (10/26/2023), Child #20 had an immunization page dated - 04/19/2023.
2. Based on the information on file, Child #20 was under the age of two years on the date of inspection.
Plan of Correction: An updated immunization page will be obtained and placed on file.
Standard 8VAC20-780-160-C
Based on review, 2 of 12 staff files indicated that it has been more than two years from the date of the most current tuberculosis (TB) screening or testing documentation on file (or more frequently as if recommended by a licensed physician or the local health department).

Evidence -
1. On the date of inspection (10/26/2023), the following TB documentation was observed on site - Staff #1 (dated - 06/23/2021), and Staff #7 (dated - 08/31/2021).
2. This was confirmed by the center director on the date of inspection.
Plan of Correction: Updated TB documentation will be obtained and placed on file.
