Inspector Notes

A Subsidy Health and Safety inspection was initiated on 3/6/2023 and concluded on 3/7/2023 to review supplemental health and safety requirements. There were 34 children present with 4 staff supervising.

A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.

Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard 8VAC20-790-600-D-1
Based on a review of five (5) staff records and interview, the vendor did not ensure that three (3) staff had current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care within 90 days of the date of employment or 90 days from subsidy vendor approval as required.

Evidence:
1. The records of staff #1 (date of employment: 10/20/2021), staff #2 (date of employment: 9/17/2021) and staff #3 (date of employment: 9/12/2022) did not contain a current CPR certification.
2. The vendor acknowledged that the staff were not certified as required.
Plan of Correction: Staff #1, Staff #2 and staff #3 will be attending our next CPR/FA training on Friday March 10 2023.
