Inspector Notes
A Subsidy Health and Safety inspection was initiated on 9/11/2023 and concluded on 9/12/2023 to review supplemental health and safety requirements. There were 41 children present with 5 staff supervising.
A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Based on a review of five staff records and interview, the vendor did not ensure that two staff annually completed the department's health and safety update course as required.
Evidence:
1. The records of staff #4 (date of employment: 1/24/2022) and staff #5 (date of employment: 9/17/2021) did not contain an annual health and safety update certificate.
