Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 11/14/2024. The on-site inspection began at 2:10pm and ended at 3:30pm. The inspector reviewed compliance in the areas listed above. There were 5 children present and 3 caregivers; the point total was14.The inspector reviewed 4 children?s records and 3 caregiver/household member records on-site. This inspection included document review, tour of the family day home, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
4The records for child #1 and child #2, who have been enrolled for more than 30 days, did not contain a physical exam.
The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening
Household member #1's most recent TB screening is dated 9/1/2022.
The most recent review of the providers emergency preparedness plan was dated 5/13/2023.
The record for child #1 and child #2, who are both under the age of two years, did not have updated immunizations.
