Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 11/14/2024. The on-site inspection began at 2:10pm and ended at 3:30pm. The inspector reviewed compliance in the areas listed above. There were 5 children present and 3 caregivers; the point total was14.The inspector reviewed 4 children?s records and 3 caregiver/household member records on-site. This inspection included document review, tour of the family day home, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to each child's attendance or within 30 days after the first day of attendance.

The records for child #1 and child #2, who have been enrolled for more than 30 days, did not contain a physical exam.
Plan of Correction: I know that they had a doctor appointment this week, I will ask the parents to provide documentation of the physicals.
Standard 8VAC20-800-180-A
REPEAT VIOLATION
The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening

Household member #1's most recent TB screening is dated 9/1/2022.
Plan of Correction: They haven't been living in the house. They just moved back in a few weeks ago. I will make sure they go get it done next week.
Standard 8VAC20-800-800-B
The provider shall review the emergency plan at least annually and update the plan as needed.

The most recent review of the providers emergency preparedness plan was dated 5/13/2023.
Plan of Correction: I will review and sign it today.
Standard 8VAC20-800-90-C
The family day home shall obtain documentation of additional immunizations every six months for each child under the age of two years.

The record for child #1 and child #2, who are both under the age of two years, did not have updated immunizations.
