During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 8/5/2025. The on-site inspection began at 3:00pm and ended at 3:45pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 3 caregivers; the point total was 15.
The inspector reviewed 6 children?s records and 3 caregiver/household members records on-site on 8/5/2025. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with
applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations

Standard 8VAC20-800-680-A
Caregivers shall wash their hands with liquid soap and warm running water after diapering.

Caregiver #2 was observed changing a child's diaper and did not wash their hands afterwards before changing another child's diaper.
Plan of Correction: We will review the procedures with all caregivers and will follow the appropriate diapering and hand washing procedures.
Standard 8VAC20-800-690-E
Soiled disposable diapers and wipes are required to be disposed in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal.

Caregiver #2 was placing soiled diapers and wipes directly into a plastic bag on the floor that the caregiver had to touch and open to place the soiled diapers and wipes inside. The plastic bag was observed to be open at the top.
Plan of Correction: We will start using the diaper changing trash can that is foot-operated.
