Inspector Notes

A monitoring inspection was initiated and concluded on November 18, 2021. There were three children(10pts) present, ranging in ages from 7 months to 18 months, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-111-70-A
Based on record review, three of three records reviewed did not have written acknowledgement of receipt of all required policies and procedures.
Evidence:
1. Child #1, Child #2, and Child #3's record did not have documentation of the Provisions of the Emergency Plan and the Liability Insurance Declaration in the record.
Plan of Correction: The provider will print copies of each document and have the parents to sign the forms and document the record.
