Inspector Notes
An unannounced renewal inspection was conducted today between the hours of 10:00 am through 11:15am. There were a total of four children (12points) observed with two caregivers. A total of four children's files and four staff/household member's files were reviewed. The children were engaged in the following: toy play, snack, hand washing, and other organized activities. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report. Violations
4Evidence:
1. The findings of the 09/06/2023 inspection notice was not posted in a conspicuous place on the licensed premises.
2. Caregiver #1 confirmed that the current inspection notice was not posted.
Evidence:
1. Caregiver #1 did not obtain a minimum of sixteen clock hours of training annually. Caregiver #1 obtained zero hours of the minimum sixteen hours of training. Caregiver #1 stated that they started some online training, but did not complete it.
2. Caregiver #2 did not obtain a minimum of sixteen hours of training annually. Caregiver #2 obtained zero hours of the minimum sixteen clock hours. Caregiver #2 stated that they started a 13 hour training, but did not complete the training.
3.Caregiver #3 did not obtain a minimum of sixteen hours of training annually. Caregiver #3 obtained three of the sixteen hours required annually.
Based on record review, children's records did not contain required information.
Evidence:
1. Child #1's record (Start date-03/13/2024) did not contain the name and policy number of the child's medical insurance.
2.Child #2's record (Start date-02/13/2024) did not contain documentation of written authorization if the caregiver is to administer prescription or non prescription medication.
Evidence:
1. Child #2's record (Start date-03/13/2024) did not contain documentation of the parent's written acknowledgement of the receipt of policies for administering medication, provisions of the family day home's emergency preparedness and response plan, and the liability Insurance Declaration.
