Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on October 3, 2025, as a part of the licensure period. The on-site inspection began at 9:50am and ended at 10:35am. The inspector reviewed compliance in the areas listed above. There were two children present and two caregivers; the point total was six. The inspector reviewed two children?s records and four caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and household member a current Report of Tuberculosis Screening form every two years from the date of the first screening.

Household Member #1 and #2 had an expired Report of Tuberculosis Screening form. Household Member #1 and #2's report was overdue by 26 months.
Plan of Correction: Household Member #1 and Household Member #2 will schedule an appointment to have a current TB screening completed.
