Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated and concluded on March 1, 2022. The provider was contacted by telephone and a virtual inspection was conducted. There were one child present, three years of age, with one provider supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of one child record and one provider?s record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
3Evidence:
The record for Child #1 (first day of attending FDH: 01/18/2021) did not contain documentation of a physical exam. The provider stated she could not find the physical exam.
Evidence:
The record for Child #1 (first day of attending FDH: 01/18/2021) did not contain the address for one of the two designated persons listed to contact in case of an emergency if the parent cannot be reached. The record for Child #1 did not contain parent's signed acknowledgement of receipt regarding provisions of the family day home's emergency preparedness and response plan.
The licensing inspector will send the "Provisions of the Emergency Preparedness and Response Plan" form for the parent to sign and add to the record.
Evidence:
The provider stated the fire drills that had been completed in 2022 had not been documented but has been completed.
