During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 12/11/2024, as a part of the licensure period. The on-site inspection began at 10:20am and ended at 1:15pm. The inspector reviewed compliance in the areas listed above. There were 69 children present and 26 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.

Staff #4's updated screening was due 2 months ago and the updated screenings have not yet been completed.
Plan of Correction: Administrative staff will request staff complete TB screening in the next 10 days.
Standard 8VAC20-780-260-B
Annual approval from the health department shall be provided, or approvals of a plan of correction, for meeting requirements for water supply and sewage disposal system.

The center had no record of Environmental Sanitation Inspection from the health department.
Plan of Correction: Center Director to contact local health department to schedule inspection, today.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

Several containers of soap with warning labels, ?Keep out of reach of children,? were unlocked in the Pre-K classroom, within reach of children.
Plan of Correction: The hazardous items were relocated to a closet that locks. A maintenance request has been put in for repair of lock on cabinet.

The center shall meet the proof of child identity and age requirements as stated in
? 22.1-289.049 of the Code of Virginia, within seven business days of initial attendance.

Child #4 attended the center for 43 days before the center viewed required document.
Plan of Correction: Administrative staff will ensure all required documentation is reviewed/received within required time frame of a child attending.

Injury prevention procedures shall be updated at least annually based on documentation of injuries and a review of the activities and services.

The center did not have annual update for injury prevention procedures.
Plan of Correction: The center will review injuries and develop injury prevention plan to review annually.

Shelter in place procedures shall be practiced a minimum of twice per year.

The center had no record of shelter in place practice drills during the past year.
Plan of Correction: Center staff to conduct 2 Shelter in place drills by the end of December 2024 and two times a year thereafter.

REPEAT VIOLATION
When food is brought from home, the food container shall be clearly dated.

None of the labels on multiple foods brough from home by a child in one classroom, were dated.
Plan of Correction: Staff corrected and dated food brought from home, during the inspection. Staff will encourage parents to write date on labels or will date labels upon receipt of food brough from home.
