During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the program was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site focused investigation was conducted on 3/18/25 in response to allegations of non-compliance in an infant room. The on-site inspection began at 1:45pm and ended at 2:38pm. The inspector reviewed compliance in the areas listed above. The inspection included document review and interviews. Evidence collected resulted in a valid finding and additional violations not originally reported were determined to be out of compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/1/25. A POC submitted after this date will not appear on the public website.

The license and any other documents required by the Superintendent, to include the most recent Inspection Summary and Violation Notice, shall be posted in a conspicuous place on the licensed premises.

The inspection report from the most recent inspection, 12/11/24, was not posted in the facility. The Inspection Summary only from the 3/28/24 inspection was posted.

Standard 8VAC20-780-350-B-1
The classroom ratios for children birth-16 months shall not exceed a staff:child ratio of 1:4.

On 3/11/25, an infant age classroom (birth-16 months of age) exceeded the maximum ratio of 1 staff to 4 infants. According to child attendance and staff sign-in records, from 10:00-10:26am there was 1 staff with 5 infants.
Plan of Correction: All staff will be retrained on signing in and out each time they enter and exit a classroom. This will ensure the number of staff listed on the attendance record matches the number of staff in the classroom. Members of the admin team will do periodic checks to ensure accuracy and compliance. Immediate.
Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

According to classroom attendance records, on 3/11/25, the attendance for 2 infant classrooms did not include documentation of the arrival and departure of each infant/child as it occurs.
Plan of Correction: All staff will be retrained on signing children in and out upon arrival and departure. Members of the admin team will do periodic checks to ensure accuracy and compliance. Immediate.
