Inspector Notes

An unannounced inspection was conducted on 8/19/25 in response to a complaint received by the licensing office on 8/5/25 relating to allegations of an unsafe area in the physical plant and inadequate staffing and supervision. This inspection included document review, tour of the infant rooms and interviews. The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/18/25. A POC submitted after this date will not appear on the public website.

The staff-to-child ratios for children ages birth-16 months are required to be maintained at 1 staff person to 4 infants whenever children are in care.

According to child and staff attendance records, on 8/4/25, 2 of 3 infant classrooms exceeded the 1:4 ratio requirement for varying periods of time.

Infant Room B: Between 9:32am-10:23am, the ratios were 1 staff:5 infants.

Infant Room C: Between 1:30pm-2:30pm the ratios were 1 staff:6 infants.
Plan of Correction: A new classroom log was designed to make recording of child and staff attendance easier to show/read. All staff were trained in the new log and admin are regularly checking/reviewing logs. Log was created immediately and staff were retrained during PDD-8/27/25-8/29/25

The center shall develop and implement a written policy and procedure that describes how the center will ensure that each group of children receives care by consistent staff or team of staff members.

The center's written policy and procedures for maintaining ratios and group size does not include provisions for ensuring that each group of children receives care by consistent staff.
Plan of Correction: The center currently assigns each child to a dedicated homeroom with dedicated classroom teachers. Support staff and/or members of the local team may step in as needed when a dedicated staff member is out or to assist with lunch breaks to ensure ratios are being maintained The center will update their written policies and procedures to reflect this. The center will submit an updated policy to licensing within two weeks. Once it has been approved, we will immediately update our handbook. [sic]

With a parent's written permission and a written assessment by the program director and program leader, a center may choose to assign a child to a different age group if such age group is more appropriate for the child's developmental level. A center may temporarily reassign a child from there regular group/class and staff but not casually disrupt a child's schedule and attachment to staff, or for the purpose of maintaining ratios.

On 8/4/25, 3 of 14 infants in attendance were moved from their assigned classroom and staff without parent written permission and a written assessment by the program director and program leader.

Child #1 was moved twice during the day and was in 2 different classrooms.

Child #9 was moved twice during the day and was in 2 different classrooms.
Plan of Correction: In order to maintain ratio on 8/4/25, children were moved within the infant suite to maintain ratio throughout the day. Going forward, if a child needs to be temporarily moved outside of drop off and pick up when classes may be combined, the center will obtain the parent's written permission and a written assessment will be conducted by the program director and/or program leader.
