Inspector Notes
An unannounced inspection was conducted on 8/19/25 in response to a complaint received by the licensing office on 8/5/25 relating to allegations of an unsafe area in the physical plant and inadequate staffing and supervision. This inspection included document review, tour of the infant rooms and interviews. The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Submit your POC within five (5) business days from today, which will be the close of business on 9/18/25. A POC submitted after this date will not appear on the public website.

Violations
3According to child and staff attendance records, on 8/4/25, 2 of 3 infant classrooms exceeded the 1:4 ratio requirement for varying periods of time.
Infant Room B: Between 9:32am-10:23am, the ratios were 1 staff:5 infants.
Infant Room C: Between 1:30pm-2:30pm the ratios were 1 staff:6 infants.
The center's written policy and procedures for maintaining ratios and group size does not include provisions for ensuring that each group of children receives care by consistent staff.
On 8/4/25, 3 of 14 infants in attendance were moved from their assigned classroom and staff without parent written permission and a written assessment by the program director and program leader.
Child #1 was moved twice during the day and was in 2 different classrooms.
Child #8 was moved twice during the day and was in 3 different classrooms.
Child #9 was moved twice during the day and was in 2 different classrooms.
