During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the program was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site focused monitoring inspection was conducted on 11/12/25, as a part of the terms of the probation period. The Department issued a Special Order, on 8/11/25, to place the licensee on probation for a 12-month period and require the licensee to contact parents, guardians or other responsible persons regarding high-risk health and safety violations. The on-site inspection began at 10:03am and ended at 11::50am. The licensing administrator was on-site today. The inspector reviewed compliance in the areas listed above. There were 23 children present and 8 direct-care staff. The inspector reviewed 2 children?s records and 2 staff records while on-site. This inspection included document review, tour of the facility and interviews. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
