The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include documentation of the time of the injury, how the parent was notified and a minimum of 2 signatures on the report.
Of 7 written injury reports reviewed, 1 report did not document the time of the child's injury, a 2nd report did not document how the parent was notified, and a 3rd report did not include a minimum of 2 signatures.

Plan of Correction: Effective immediately, all injury reports will be reviewed by the Director and designated administrator to ensure completeness and compliance. Staff will receive refresher training on proper completion with emphasis on required fields and signatures.
