This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a tour of the program area.

An initial monitoring inspection was initiated on October 23, 2021 and concluded on October 23, 2021. The provider was contacted by telephone and a in person inspection was conducted. The inspector reviewed compliance in the areas of administration, staff training and qualifications, physical plant, programming, special care and emergencies and special services.

The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
