Inspector Notes

An unannounced monitoring was conducted on April 26, 2023 from approximately11:00am- 1:30pm. There were 42 children present upon arrival with 8 staff members. Additional administrative staff were present. Children were observed freely playing and taking nap. .Four staff records and 4 children records reviewed . Licensing Inspector reviewed indoor areas, supervision, activities, equipment, emergency evacuation and sheltering-in-place drill documentation, emergency procedures and required posting.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and staff interview, the licensee did not ensure the documentation for tuberculosis was completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence:
Staff#2 hire date was November 11, 2022 and TB test was completed November 17, 2022.

Staff #3 hire date was November 22, 2022 and TB test was completed December 1, 2022.

Based on observation, the licensee did not ensure nonabsorbent surface for diapering or changing was used.

Evidence:
In the infant room, their was a multiple small tears on the diapering surface causing the surface being non-absorbant.
In the girls bathroom, their was a tear on the diapering surface causing the surfacing being non-absorbant.

Based on record review and staff interview, the licensee did not enure Lockdown procedures were practiced at least annually.
