Inspector Notes

An unannounced complaint inspection was initiated on September 28, 2023 from 2:00 pm -3:34pm regarding concerns of administration and Staffing and Supervision. During the inspection the following were completed: interviews, a review of records/documentation and observation of the classroom . The preponderance of the evidence gathered during the investigation does support the allegation related to Staffing and Supervision; therefore, the complaint is determined to be VALID.

The inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on interviews and document review, the licensee did not ensure when staff are supervising children, they shall always ensure their care, protection and guidance.

Evidence:

1.Child #1 fell asleep in a rocker . Staff #1 picked the rocker up with child #1 in it. Staff #1 moved the rocker with child#1 asleep and placed it in the crib with a mattress.

2.Staff #1, Staff #2 and Staff #3 confirmed child #1 was in the rocker placed in the crib while sleeping .

3. Manufacture guidelines for the infant rock seat state the following:
- Never move the unit while child is in the seat.
- Always use on the floor.
- Never use on soft soft surface as seat may tip over and cause suffocation.

4. The center did not follow the manufacture guidelines.
