Inspector Notes

A unannounced mandated monitoring inspection was initiated on April 17, 2024 from 9:00 am- 11:11am. There were 36 children with 7 staff present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming,medication,
special care and emergencies and nutrition. A total of five children records and six staff records were reviewed. The children were observed freely playing. The outside areas were inspected.
Center requested to have measurements and floor plan to be reviewed during inspection .

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
Based on record review and staff interview, the licensee did not ensure to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:
Staff #2 lived in Minnesota with in the last five years. Staff #2 hire date was 08/28/2023.
Staff #2 did not obtain results of sex offender and child abuse and neglect.
Plan of Correction: Director will keep up to date records and review records evey 3 months.

REPEAT
Based on observation, the licensee did not ensure at least every two years from the date of the first initial screening.

Evidence:
Staff #6 most recent TB test 7/29/2021.
Staff #5 most recent TB test was 11/18/2019.
Plan of Correction: Center director will keep updated records and will review records every 3 months.
Standard 8VAC20-780-240-A
Based on record review, the licensee did not ensure the shall be completed within 90 calendar days of employment.

Evidence:
Staff #3 hired on 10/09/2023 and and did not complete the Virginia Department of Education-sponsored orientation course with in 90 days of employment. Staff #3 completed training on 1/24/2024.
Plan of Correction: Director will keep up to date records and review them every 3 months.
Standard 8VAC20-780-330-B
Based in staff interview and observation the licensee did not ensure where playground equipment is provided surfacing shall comply with the minimum safety standards and loose resilent surfacing should be under equipment with moving parts or climbing apparatus to create a fazzl zone free of hazordous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface.

Evidence:
There were two merri-go-round bikes , small slide, and spring equipment that did not have resilent surfacing. Center director confirmed the equipment was being used.
Plan of Correction: Director will have resilent surfacing put down within the next 30days.

Based on observation, the licensee did not ensure the food brought from home is labeled date and child's name.

Evidence:
Two lunch boxes were observed not labeled with date.
Plan of Correction: Will retrain staff on labeling date and name on food brought from home.

Based on observation, the licensee did not ensure children should be allowed to drink or eat while walking around.

Evidence:
Child #1 was walking around in the classroom with a bottle.
Child #2 was walking around down the hallway drinking out of a sippy cup.
Plan of Correction: Will not have children walking while drinking and will have children to sit at a table.

Based on observation, the licensee did not ensure bottles were labeled with child's name and dated.

Evidence:
Child #1 was walking around the classroom with a bottle not labeled with name and date .
Plan of Correction: Staff will makesure all bottles will be labeled with date and name.
