Inspector Notes

An unannounced renewal inspection was conducted from 11:15 am - 12:30 pm with the provider and her assistant. There were five daycare and two household children, ranging from nine months to three years of age, for a total of 19 points. Children were observed playing with a variety of age-appropriate toys. Two child records, three caregiver/household member records, the emergency drill log, emergency supplies, daily attendance, and required posted information were all reviewed. The provider is not currently certified in Medication Administration Training (MAT) and cannot administer medication to children in care. If you have further questions about this inspection please contact Donna Liberman at 540-359-5244 or Donna.Liberman@doe.virginia.gov.

Violations

1
Standard 8VAC20-800-100-A
Based on observation, and an interview, the provider did not obtain documentation of a physical exam by or under the direction of a physician before the child's attendance or within 30 days after the first attendance. Evidence: the record for child #2 (date of first attendance: 2/23/23) did not contain documentation of a physical.
Plan of Correction: Will obtain documentation of a physical.
