Inspector Notes

An unannounced monitoring inspection was conducted from 10:45 am - 12:15 pm with the provider and her assistant. There were three daycare and two household children, ranging from 22 months to four years of age, for a total of 11 points. Children were observed playing with a variety of age-appropriate toys. Two child records, three caregiver/household member records, the emergency drill log, daily attendance, and required posted information were all reviewed. The provider is not currently certified in Medication Administration Training (MAT) and cannot administer medication to children in care.

(Repeat violation) Based on observation, and an interview, the provider did not obtain documentation of a physical exam by or under the direction of a physician before the child?s attendance or within 30 days after the first attendance. Evidence: the record for child #1 (date of first attendance: 7/9/23) and child #2 (date of first attendance: 6/7/22) did not contain documentation of a physical.

Based on review of three provider/household member records, and an interview with the provider, not all providers/household members had a current Tuberculosis (TB) screening/test every two years as required. Evidence: the record for caregiver #1 and household member #2 had TB tests/screenings dated: 7/28/2021.

Based on observation, the provider failed to ensure that all small electrical appliances are unplugged unless being used by the provider. Evidence: there was a toaster oven and a coffee maker sitting on the kitchen counter plugged in.
