Inspector Notes
A subsidy health and safety (SHSI) inspection was conducted on 12/14/2023. There were three daycare and two household children present with the provider and an assistant supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Based on review of documentation, and an interview with the provider, it was determined that the provider does not have an emergency preparedness plan that addresses procedural components for lockdown procedures. Evidence: the provider's emergency preparedness plan does not include lockdown procedures.
