During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was initiated on9/30/2024 and completed on 9/30/2024. The on-site inspection began at 11:45am and ended at 1:15pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 3 caregivers; the point total was 28. The inspector reviewed 6 children?s records and 4 caregiver/household member(s) records on-site on 9/30/2024. This inspection included document review, a tour of the facility, observations and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/10/2024. A POC submitted after this date will not appear on the public website.
Violations
6One crib had mesh sides around and above the crib; three cribs had mattresses that did not fit snugly next to the crib.
Two children were observed sitting in a high chair for 55 minutes.
The parents of Child #2 were not provided in writing the following required information: provisions of the family day home?s emergency preparedness and response plan. Child #2 has been in care for a year and nine months.
Three topical skin products were not labeled with a child?s name.
Three topical skin products were sitting in a basket on a bottom shelf, accessible to children in care.
There have been no documented evacuation drills for four months.
