Inspector Notes

An Initial inspection was initiated on September 23, 2021 and concluded on September 29, 2021 to determine compliance with licensing standards. The inspection was conducted on site with the director. The facility plans to provide care to children four years, nine months through age 12 years. Measurements of the facility were calculated along with review of the certificate of occupancy to determine a capacity of 120 children. The facility is equipped with a variety of age and stage appropriate supplies and materials. Snacks will be provided by the facility and an evening meal will be provided by Feed More. Prescription medications will be administered with proper authorization. Staff have MAT certification. Standards related to supervision and program were not determined today. The program is known to the Department and the director was knowledgeable of the standards. A conditional license will be recommended. An unannounced follow-up inspection will be conducted within 60 days to determine compliance with standards not determined at the initial inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, the facility did not ensure all hazardous substances were kept in a locked place.

Evidence:

1. The supply closet in the pavilion contained cans of house paint. The closet was unlocked.
2. The director stated the lock on the closet door was broken.
Plan of Correction: Per the director: The paint will be removed today and we have requested the lock be repaired on the door.
