An unannounced monitoring inspection was conducted today.
There were 28 (ages 3 months-4 years) children present with 13 staff.
The children were having eating breakfast, coloring, drawing, and playing outdoors.
Three child records, and six staff records, were reviewed.

Based on a review of staff files, the facility failed to ensure that all employees complete a fingerprint background check prior to the first day of employment.
Evidence:
1. There is no fingerprint background check on file for staff #5, 6. Hire dates 9-5-23, 9-14-23.
Plan of Correction: The human resource director will ensure that all employees complete all required background checks.
Standard 8VAC20-770-60-C-2
Based on a review of staff files, the facility failed to ensure that all employees have a central registry clearance on file within 30 days of hire.
Evidence:
1. There are no central registry clearances on file for staff #1, 2 & 4. Hire dates 5-31-23, 7-24-23 & 6-5-23.
Plan of Correction: The human resource director will ensure that all employees complete all required background checks.

Based on a review of the outdoor play area, the facility failed to ensure that all ground supports are covered with materials that protect children from injury.

1. The concrete footer for a post on the climber is accessible to children.
