During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated on 03/11/2025 and completed on 04/04/2025 with local agencies in response to a self-report received by the licensing office on 03/11/2025 relating to supervision and care of children. The inspector reviewed 9 children?s records and 10 staff records. This inspection included document review Injury logs, policies and procedures, tour of the facility, interviews, observations, and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self-report, violations that were not reported were found are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/18/2025. A POC submitted after this date will not appear on the public website.
Violations
7Staff #1, #2, #3, #4, #5, #6, #7, and #8 became aware of reportable offenses, and did not report it to the local department or to the Department's toll-free child abuse and neglect hotline for over 60 days.
There was no physical exam records for Child #7 who had been in care for over 5 months.
The following actions or threats thereof are forbidden: physical punishment, striking a child, roughly handling a child, restricting movement through tying, and forcing a child to assume an uncomfortable position.
From December 2024 through March 2025, Staff #9 and Staff #10 spanked and smacked Child #1, #2, #3, #6, #7, #9 on the hands, mouth, or bottom.
From December 2024 through March 2025, Staff #9 and staff #10 tripped Child #1, #2, #3, and #6. At times children fell down as a result of the tripping.
From December 2024 through March 2025, Staff #9 and #10 threw shoes at Child #1, #2, #3, #4, #6, #7, #8, and #9. Staff #9 threw a shoe at Child #6 but missed and hit Child #5 (age 4 yeas old) on the side of the head.
From December 2024 through March 2025, Staff #9 and #10 would buckle Child #1, #2 #3 and # 7 in a feeding chair nine inches high from the floor that has a tray to discipline them and at times would tip the chair back wards while they were seated.
From December 2024 through March 2025, Staff #9 and #10 sat on Child #1, #3, and #9. The children would be on their stomachs and the staff would sit on their backs.
The following actions or threats thereof are forbidden: punishment by another child.
Staff #9 and #10 over a two -week period in March 2025 told children to point and say to other children "shame" or "bad." Staff #9 would tell other children to hit and kick child #3.
The following actions or threats thereof are forbidden: Verbal remarks that are demeaning to a child.
Staff #9 and staff #10 made verbal remarks to child #1, #2,#3, #4, #6, #7,#8, #9 that are demeaning to a child.
The following actions or threats are forbidden: applying unpleasant or harmful substances.
Over a 2-month period in 2025, Staff #9 applied soap into the mouth of Child #6 (5 years old).
Over a 2-month period in 2025, Staff #10 applied soap into the mouths of Child #1 (age 4 years old) and #7 (3 years old).
The provider shall maintain and keep at the center a separate record for each child enrolled which shall contains all of the required information.
1. Children's record for #5 did not contain emergency addresses for two contacts emergency contacts and had been enrolled for over 6 months.
2. Child record #2 did not contain two emergency addresses and had been enrolled over 6 months.
3. Child record #1 (enrolled over 6 months), #2 and #5 did not contain information about any chronic physical problems and pertinent developmental information and any special accommodations needed.
