Inspector Notes

An unannounced complaint inspection was initiated on 4/21/25 and completed on 05/02/25 in response to a complaint received in this office on 04/10/25 relating to supervision of children. The inspector reviewed 10 children?s records and 1 staff record on site. This inspection included document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original complaint, a violation that was not reported was found relating to staff qualifications and are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-D
In each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present.

Staff #1, who did not meet lead teacher qualifications, was the lead teacher in the infant room with four infants under the age of 14 months. Staff #1 had been actively working at the center for the past two and a half months.
Plan of Correction: Staff #1 was assigned to a different classroom as an aide and replaced with a qualified lead teacher.

The required ratio for birth to 16 months is one staff per 4 children.

Staff #2 was alone with 5 infants for approximately 8 minutes.
Plan of Correction: A staff person called in to say she would not be at work. A staff person was called into work. Staff #3 went and got children form the infant room so that the teacher would have the 4:1 ratio.
