During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during a previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site focused monitoring inspection was initiated on 7/29/25 and completed on 7/29/2025. The Department issued a Notice of Intent to Revoke the License on April 30, 2025, as a result of high-risk violations.
The inspection began at 10:10 and ended at 11:55. The inspector reviewed compliance in the areas listed above. There were 8 children and 5 staff present. The inspector reviewed 5 children's records and 6 staff records. This inspection included document review, tour of the facility, interviews, and observation.
Information gathered during the inspection determined non-compliance with the applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1Each center shall maintain at the center a separate record for each child enrolled that contains all required information.
The record for child #5, enrolled for 6 months, was missing information on any chronic physical problems, and pertinent developmental information and any special accommodations needed.
