Inspector Notes
The Department entered into a Consent Agreement with the facility on 08/12/2025. An unannounced on-site monitored inspection was initiated and completed on 9/23/2025. The inspection began at 10:45 and ended at 11:50. The inspector reviewed compliance in the areas listed above. There were 8 children and 4 staff present.. The inspector reviewed 4 children?s records and 4 staff/licensee/administrator records. This inspection included document review, tour of the facility, interviews, and observation.
Information gathered during the inspection determined non-compliance with the applicable standards or law, and violations are documented on the violation notice issued to the program.
