Inspector Notes
The Department entered into a Consent Agreement with the center on 08/12/2025. An unannounced on site inspection monitoring inspection was initiated and completed on 12/16/2025 by two inspectors. The on-site inspection began at 1:00 and ended at 1:40. The inspector reviewed compliance in the areas listed above. There were 7 children and 3 staff present. The inspector reviewed two staff records. This inspection included document review, tour of the facility, interviews, and observation.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
