During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced investigation was initiated on September 3, 2025, and completed on September 4, 2025, with local agencies in response to a complaint received by the licensing office on August 27, 2025, relating to allegations of forbidden actions. The inspector reviewed one child's record, and one staff record electronically on September 16, 2025. This inspection included document review, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Forbidden actions include physical punishment, striking a child, roughly handling or shaking a child, restricting movement through binding or tying, forcing a child to assume an uncomfortable position, or exercise as punishment.

Video footage from an elementary school on August 26, 2025, at approximately 3:25pm, shows Child #1 (age: 7 years) walking out of the school and Staff Member A using her hand to slap Child #1 on the back of the head.
Plan of Correction: Retrained all staff when doing a head count during a transition from one location to another, doing so without touching the children?s head in any way. (i.e. using high fives, fist bumps, shoulder touches, etc.)

Administered additional training for entire staff on all forbidden actions under standard. Thorough training included complete review of all forbidden actions and review of detailed examples of each action.

The staff member was put on leave pending the investigation of the off-site allegation and ultimately separated from the company.
