Inspector Notes
A renewal inspection was conducted on 5/3/2022. 15 children were directly supervised by 4 staff members. The physical space, attendance, evacuation drills, injury reports, staff records, orientation procedures, parent/staff handbooks, behavioral guidance policies, 3 children's records, medications and required posted documents were reviewed. Standard 8VAC20-780-500-A
Based on observation, disposable diapers were not disposed of in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal [8VAC20-780-500.B.5]. Evidence: The trashcan next to the changing station was not covered with a foot-operated system to ensure proper disposal of diapers.
Plan of Correction: We will order a new trashcan right away.

Based on record review and interview with the director, documentation was not maintained for emergency evacuations. Evidence: Evacuation drill details for 2 out of the last 4 months were not written on the log.
Plan of Correction: We did the drills but forgot to write the information on the log. We won't forget moving forward, setting up a monthly reminder.

Based on record review, "future action to prevent recurrence of the injury" was not included on the center's injury reports. Evidence: On 3 out of 3 injury reports reviewed, a future action to prevent recurrence of the injury was not listed on the report.
Plan of Correction: We will review the standard to make sure all components are included on our injury reports.
