Inspector Notes

A Subsidy Health and Safety inspection was conducted on September 14, 2022, to review supplemental health and safety requirements. There were 4 children under the supervision of 2 staff.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the Violation Notice.

Based on staff records review, staff did not have documentation of tuberculosis screening at the time of employment and prior to coming into contact with children.
Evidence:
1. Staff #1 and #2 did not have a documentation of tuberculosis screening at the time of employment and prior to coming into contact with children.
Plan of Correction: As a religious exempt program we only request Staff Health forms to be completed for staff. We recently applied as a subsidy provider and we were not aware of a TB screening had to be completed for staff. It was not listed in the subsidy application. Staff will submit TB screenings on Friday, September 16, 2022. We will request new staff members to have TB screening prior to employment.

A Subsidy Health and Safety inspection was conducted for the Vendor on September 14 2022 however, the vendor?s subsidy application was denied approval on September 13, 2022 because TB screening results were not submitted. Since the inspection was already scheduled before the denial was sent to the vendor, the scheduled inspection took place even though the vendor?s application had not been approved.
