Inspector Notes

A (monitoring/renewal/complaint) inspection was conducted on April 17, 2023. There were twenty-eight children present with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and two staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, facility did not ensure hazardous substances such as cleaning materials are kept in a locked place using a safe locking method that prevents access by children.

Evidence: Cleaning supplies were observed stored on top of a refrigerator in the cafeteria. Hand sanitizer was observed stored on a table in the cafeteria.

Based on review, facility did not ensure 2 of 5 children's records have documentation of required information.

Evidence: The following was found during record review:
1. The record for child #3 (start date 12/4/2022) did not have parent employment information and emergency contact addresses.
2. The record for child #5 (start date 3/23/2023) did not have parent's employment phone number and emergency contact addresses.
