Inspector Notes
An announced initial inspection was initiated on 12/28/2021 and concluded on 12/28/2021. The physical plant, classrooms and materials, and emergency supplies were inspected. 4 staff/agent records were reviewed. No children were present during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, the center did not provide resilient surfacing under equipment with moving parts and climbing apparatus. Fall zones did not encompass sufficient area to include the child's trajectory in the event of a fall.
Evidence:
1. The playground area contained equipment to include a slide, swing set, and jungle gym. The playground area did not contain resilient surfacing.
2. The jungle gym did not have a fall zone area of 6 feet around all sides as required; the fall zone contained a column from a building on one side.
