Inspector Notes
An unannounced monitoring inspection was conducted on November 30, 2023, from 9:20am-12:40pm. At the time of entrance, (21 ) children were in care with ( 4 ) staff members present. The inspector reviewed 7 children?s records and 5 staff records. Children were observed during snack time, engaged in toy play, coloring and outdoor play time. Medications were observed. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.
Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

7Based on staff record review, the center did not obtain a Sworn Statement of Affirmation prior to the staff's employment.
Evidence: Staff # 5, date of employment 12/21/2022 does not have documentation of a Sworn Statement of Affirmation. The staff was observed caring for children this date.
Based on a review of staff records, 2 staff do not have documentation of TB screening at the time of employment or within 30 days of the date of employment.
Evidence: Staff # 4, date of employment 11/29/2023 and staff # 5, date of employment 12/21/2022 does not have documentation of TB screening. Both staff were observed this date supervising children.
Based on staff record review, 1 staff has not completed the Virginia Department of Education orientation course within 90 calendar days of employment.
Evidence: Staff # 5, date of employment 12/21/2022 does not have documentation of the 10 hour Virginia Department of Education orientation course.
Evidence: Staff # 3, date of employment 7/19/2022, staff #4 date of employment 11/29/2023 and staff #5 date of employment 12/21/2022has not completed the required training on recognizing child abuse and neglect.
Evidence: Child # 3 has a emergency medication, the medication authorization form is completed by the parent but not by the physician.
Evidence: Child # 2, date of first attendance 12/5/2022 does not have documentation of the child's physical and immunization record.
Evidence: staff # 4, date of employment 11/29/2023 does not have documentation of 2 references before employment. The staff was observed supervising children this date.
