During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was completed on 10/15/2025, as a part of the licensure period. The on-site inspection began at 9:20 am and ended at 10:45 am. The inspector reviewed compliance in the areas listed above. There were (28) children present and (6) staff. The inspector reviewed (5) children?s records and (5) staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.
There was no physical examination in the record of child #1, who had been enrolled for 36 days.
Plan of Correction: We will ask the parent for the physical examination.
Standard 8VAC20-780-240-A
Staff are to complete the VDOE sponsored online orientation course within 90 days of hire. Staff #3, who is working with children, has not yet completed the 10 hour course which is overdue by 10 months.
Plan of Correction: Staff will take the 10 hour training.
Standard 8VAC20-780-240-B
Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Staff # 3 has not completed the training on recognizing child abuse and neglect. Staff was working with children this date and the training is overdue by 1 year.
Plan of Correction: Staff will take the child abuse and neglect training.
Standard 8VAC20-780-245-J
Whenever the center agrees to administer prescription medication, a staff member must have completed a medication administration training program.
There was 1 child that was in attendance this date that had emergency medication at the center. There was no staff present that was trained in the medication administration training(MAT) this date.
Plan of Correction: Director will have other staff take the MAT training to allow for a staff present at all times that has MAT training.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.
There was no record for Child #5, enrolled for 2 days.
Plan of Correction: We will get the record for this child.
