Inspector Notes
An unannounced renewal inspection was conducted on June 4, 2024, from 9:15am-11:15am. At the time of entrance, 4(8pts) children were in care with one caregiver present. The inspector reviewed 4 children?s records and 4 /staff/household member's records. Children were observed during toy play, puzzles, engaged in group activities and outdoor play time. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.

1. The family day home did not have the findings of the most recent inspection posted. The findings posted was dated 05/31/2023. The last inspection was 03/19/2024.
2. Caregiver #1 confirmed that the findings for the most recent inspection was not posted.
Evidence:
1. Child #1's record (Start date-10/30/2023) did not contain documentation of a physical examination by or under the direction of a physician.
Based on record review and interview, children's records did not contain required information.
Evidence:
