Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on November 21, 2024, as a part of the licensure period. The on-site inspection began at 8:45 and ended at 9:45am. The inspector reviewed compliance in the areas listed above. There were four children present and one caregiver; the point total was eight. The inspector reviewed four children?s records and four caregiver/household member(s) records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

There was no physical exam record available for Child #3, who has been in care for over a year.
Plan of Correction: The provider contacted the parent to obtain a copy of the current physical.
Standard 8VAC20-800-140-B-3
The provider shall have current certification in cardiopulmonary resuscitation (CPR) as appropriate to age of the children in care issued within the past two years.

Current certification in CPR was not current for Staff #1. Certification of CPR expired 23 days ago.
Plan of Correction: The provider will schedule an appointment to complete CPR training.

Poisonous materials shall be stored in an area inaccessible to children.

A container of deodorizer with the warning of Keep out of reach of children was on the sink area in the bathroom that the children use.
Plan of Correction: The container of deodorizer was locked up at the time of the inspection.
Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required in each child's record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review.

Child #3's record did not contain a parent's signature for an annual review of the emergency contact information. Child #3 has been in care for over a year.
Plan of Correction: The parent will review the emergency contact information and sign the child record as acknowledgement.
Standard 8VAC20-800-90-C
Documentation of additional immunizations is required once every six months for children under the age of two years.

Child #3, 25 months old, last documented immunization was over a year ago.
Plan of Correction: The provider contacted the parent to obtain updated immunizations from the doctor.
