Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on May 7, 2025, as a part of the licensure period. The on-site inspection began at 10:00am and ended at 11:00am. The inspector reviewed compliance in the areas listed above. There were two children present and one caregiver; the point total was four. The inspector reviewed four children?s records and four caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-60-B
Children's records are required to contain the policy number of the child's medical insurance.

The records for Child #3 and Child #4 were missing the policy number of the children's medical insurance. The children were not present during the inspection and have been in care for 19 days.
Plan of Correction: The provider contacted the parent of the Child #3 & Child #4, siblings, to obtain the policy number of the medical insurance. Moving forward, the provider will ensure to check the children's records for completion.

Repeat Violation

Annually, the provider shall review with the parent the emergency contact information required in each child's record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review.

Records for Child #1 and Child #2 did not have the parent's signature for annual review of the emergency contact information. Child #1 and Child #2 had been in care for over a year without any annual review.
Plan of Correction: The provider will have the parent of Child #3 & Child #4, siblings, review the emergency contact information and sign as acknowledgment of the review. Moving forward, the provider will create a document that will have the dates for the annual review of each child in care.
