Inspector Notes

An in-person monitoring inspection was conducted on April 27, 2022. There were 2 children present with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2child records and 1 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the provider did not maintain an up to date record for each enrolled child.

Evidence: Child 1 did not have a complete record and was information including but not limited to guardian work information, two emergency contacts, doctor name and phone number.
Plan of Correction: I will make sure to get the information in the file.
