Inspector Notes

A focused monitoring inspection was initiated on Monday, August 22, 2022 and concluded on Tuesday, August 23, 2022. There were 14 children present, ranging in ages from 7 months to 3 years old, with 5 staff supervising including administrative staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care, and background checks. Based on review of staff records and interview, the center did not ensure to obtain documentation of tuberculosis screening within the last 30 calendar days.

Evidence: In Staff#3's record (date of hire 07/06/22), the TB screening provided for inspection was dated 05/07/22. Interview with staff confirmed the most recent TB screening available for review.
Plan of Correction: The center will ensure all TB screening requirements for staff are followed and available in staff records for review.

Based on observation, the center did not ensure all hazardous substances are kept in a locked place using a safe locking method.

Evidence: In the Infant room, there was a bottle of multi-surface cleaner and hand sanitizer observed on the shelves unlocked with the labels "keep out of reach of children" and "warning".
Plan of Correction: The center will retrain staff on hazardous substances to ensure all are locked and inaccessible to the children in care as required.
