Inspector Notes

A monitoring inspection was completed on March 6, 2023. There were 13 children present with 4 staff supervising and including administrative staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of staff records and interview, the center did not ensure to obtain a copy of the results of a search of an out of state child abuse and neglect registry.

Evidence: In Staff#1's record (date of hire: 09/13/22), there was no results of an out of state central registry found. Interview with staff confirmed the search results not being available for review during the inspection.

Based on observation, the center did not ensure children's hands shall be washed after toileting and any contact with blood, feces, or urine.

Evidence: During the inspection on March 3, 2023, there was observation of Staff#1 diapering a child in the restroom on the diapering area and Child#1's hands were not washed after diapering.
