Inspector Notes
A monitoring inspection was completed on February 6, 2024 at 11:00 am until 1:29 pm at the child day center. There were 15 children present with 5 staff supervising and including administrative staff. There were observations of the children in care completing bathroom break, eating lunch, transitioning into napping, engaged in active play with age appropriate equipment, diapering/feeding by staff, and interacting with staff. The inspector reviewed compliance the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. 5Evidence: In Staff#3's record (date of hire: 12/11/23), there was no documentation of a search of the child abuse and neglect registry from the state of New York.

Evidence: In Staff#2's record (date of hire: 11/19/21) the last dated tuberculosis screening or testing was dated 12/29/21.

Evidence: The center's last annual health inspection was dated 09/16/22 on the health department website. Interview with staff confirmed the documentation available for review during the inspection.
