Inspector Notes

An monitoring inspection was conducted virtually with the provider. Eleven children were present with 3 caregivers providing supervision. The ages of the children ranged from 4 months to 4 years. Two newly hired staff and four children's files were reviewed. The areas of the standards reviewed included administration, staff qualifications and training, physical plant, staffing and supervision, programs, special care provisions and emergencies and special services.
The violation issued is outlined on the violation notice.

Based on interview and a review of one staff file, it was determined that an employee ( staff A) was hired on 1/10/22 and the provider had not obtained their out of state sex offender registry background check for Hawaii, which must be obtained prior to the first day of employment.
Plan of Correction: The background check was provided to Licensing on 2/9/22. I will remember to obtain this background check before employment.
