An unannounced monitoring inspection was conducted today.
There were nine preschool children present with two staff.
The children were playing in the gym, reading stories and singing.
Two child records, and five staff records, were reviewed.

Based on the review of five staff files, the facility failed to ensure that all employees complete a central registry check within 30 days of hire.

Based on a tour of the playground, the facility failed to ensure that there are not entanglement points on the climber.
Evidence:
1. A bolt attaching the slide to the climber is not secure. This creates an entanglement point between the slide and the deck of the climber.

Based on a tour of the playground, the facility failed to ensure that the playground climber openings which allow a 3 1/2 inch by 6 1/4 inch rectangle to pass through also allow a 9 inch circle to pass through.
Evidence:
1. On the upper deck of the climber, near the roof, there are four triangular openings. These areas allow a 3 1/2 inch by 6 1/4 inch rectangle to pass through but do not allow a 9 inch circle to pass through. These openings create head entrapment points.
