Inspector Notes

An unannounced, on-site renewal inspection was initiated on 6/6/24 and concluded on 6/6/24. The on-site inspection began at 9am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 24 children present and 5 staff. The inspector reviewed 5 children?s records and 4 staff records on-site.

This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Child file #4 did not have documentation of immunizations as required; the child was in care during this inspection and for the 3 days prior to inspection.
Plan of Correction: The parent was contacted and proof of immunization will be obtained and placed in the file. Immunizations will be obtained prior to children beginning care moving forward.
Standard 8VAC20-780-160-A
Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.
Staff files #1 and #4 did not have documentation of a negative tuberculosis screening as required. Staff #1 and #4 have been actively working with the children for several months.
Plan of Correction: Staff will immediately obtain the screening. Staff will obtain screenings prior to working with children moving forward.
