Inspector Notes

Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on April 13, 2022 from 11:00 p.m. - 12:00 p.m. At the time of the arrival, there were seven children in the care of the provider and one assistant ranging in ages 9 months through 3. Points totaled seventeen. Five children records and the record for two assistants were reviewed and updated.

Areas observed were: handwashing/bathroom/diapering procedures, free play, lunch preparation and lunch. Children appeared to be aware of the daily routine and schedule. Areas of non-compliance may be found on the violation notice.

Based on record review, the provider failed to have a complete street address for one child.

Evidence:
1. Five children records were reviewed.
2. Child A did not have documentation of a complete physical address for 1 emergency contact.
3. The provider confirmed the complete address was not documented.
Plan of Correction: The parent will complete the emergency contact to include the complete physical address.
Standard 8VAC20-800-720-A
Based on review of medications, the provider failed to have one nonprescription medication labeled with the child's name.

Evidence:
1. Little Peoples Saline Nose drops was not labeled with the child's name.
2. The provider confirmed the Saline Nose drops were not labeled with the child's name.
Plan of Correction: The saline nose spray was immediately labeled with the child's name.
