Inspector Notes

The Licensing Inspector initiated and completed a monitoring inspection on 12/27/2022. The inspector was on site from 10:15am until 12:40pm.

There were thirty-seven children under the care of nine staff. The ages of the children ranged from three to twelve years old. The children were observed during movie time and group time. Four children?s record and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the center failed to renew the Central Registry check after expiration of 5 years.

Evidence: Staff #2 last Central Registry results was dated 08/21/2015. A new Central Registry check was not on file during the inspection on 12/27/2022.
Plan of Correction: New Central Registry check form will be completed and sent out by the end of the week.
Standard 8VAC20-780-160-C
Based on record review, the center failed to ensure each staff member has a documented TB test or screening every two years.

Evidence: Staff #2 was working day of inspection with an expired Tb test or screening. Staff #2 last TB screening was dated 12/2/2019.
Plan of Correction: Tb Test will be completed by first week in January.
